VISA Application Form With a Photograph (Download and fill up the following “Schengen Visa” Form (Please DO NOT leave blanksin the form. Indicate “NA” if answer is not applicable.)
PHOTO (One (1) recent passport-sized or 2x2 inch colored ICAO standard photo with white background (ears should be exposed and with collared shirt/blouse)
PHOTOCOPY OF BIOGRAPHICAL PAGE OF THE PASSPORT
Travel Documents (Old (if available) and current valid passport (expiring date must be three months longer than visa requested period. Please consider a 6-month validity as required by the Philippine Bureau of Immigration)

PROCESSING TIME
As a rule, the processing time of an admissible Schengen visa application should not exceed 15 calendar days. If further checks are necessary, the processing time may increase to 45 calendar days.
IMPORTANT REMINDERS:
Bring ORIGINALS and PHOTOCOPIES of ALL required documents
Bring both your original old and most recently issued passport
Photocopies of the first page and last page of both the old passport and most recently issued passport are required. Photocopies of previous visas and immigration stamps from countries previously visited are required
Each applicant must have their own file. Please ensure that for Families with shared documents, you must provide individual copies of documents for each member
Fill-up your application form accurately. Indicate N/A for items that are not applicable. Do not forget to affix your signature on the last box of the application form
